What is the minimum requirement of a blog for AdSense?

What are the bare minimum standards and policies my blog needs to meet – such as focused quality content, regular publishing schedule, clean layout, high traffic levels from real human visitors, and compliance with Google AdSense program rules – before I can qualify for approval to run Google AdSense ads and start monetizing my blog content through their contextual text and display advertising network?
